Normally
all check up tests are done
by prior appointment only.
Appointment can be taken either
personally or through a representative.
The health check up coordinator
and the in house Public Relations
Officer (PRO) will guide you
through the whole process.
-
Candidates are requested to
fill the appointment registration
form and submit it with necessary
documents to the health check
up coordinator.
-
Candidates are requested to
come on an empty stomach (i.e.
no liquid or solid food).
You are requested to be nil
by mouth from 10:00 pm the
previous night. Medications
should be taken as per prescriptions.
-
Please
bring your previous medical
records, details of medicines
taken and procedures undergone
to facilitate diagnosis and
further line of action.
-
Candidates are requested to
arrive well in time i.e. atleast
15 minutes before the scheduled
time to ease the process of
registration and other formalities.
-
Blood
samples for different tests
will be collected. A second
sample may be required if
any discrepancy is observed
in the first sample.
-
Fees
for the concerned programme
are also to be submitted to
the hospital at the time of
submitting forms. Your appointment
will be confirmed only when
the full fees for the package
are deposited in the hospital.

The
hospital reserves the right
of admission and also the
right of postponement of the
appointment fixed, if so required
in any emergency situation
or otherwise.

Candidates forfeit their fees
for the health check up programmes
if no information regarding
cancellation of the appointment
